CANCELLATION, FINANCIAL & SCHEDULING POLICY

Better Me Bariatrics Financial, Payment Policy

Better Me Bariatrics, LLC (Better Me Bariatrics) financial, deposit, cancellation, and refund
policy is described below and pertains to all monies and payments made to Better Me
Bariatrics, LLC including, but not limited to: credit cards, checks, wire transfers, bank deposits,
bank transfers, ACH payments (e-check), cashier’s check, cash, and all other payments made
to Better Me Bariatrics.

Deposit and Booking Surgery Policy

A deposit is a security deposit due at the time of scheduling the surgery to secure your
surgery date and to get consulted by our staff. The booking deposit will apply to the total
balance owed for your procedure.

Booking Deposits of less than $400 USD

Occasionally, we’ll accept a booking deposit of less than $400 USD. If that is made, you’re not
eligible for any refunds. A booking deposit of less than $400 USD, for example, $300 USD or
$250 USD, will be used towards your total balance – however, it’s not available for the refunds
nor cancellation policy.

Refunds and Cancellation Policy

All cancellations and refunds must be received by Better Me Bariatrics, LLC in writing, via
email or fax. Please allow up to six (6) weeks for processing refunds.

Booking Deposit Refund Policy (at least $400 USD)

Refunds and cancellations must be made in writing. Fully refundable if canceled in writing at
least 31 days before surgery. Between 32–45 days before surgery: $100 refund. At least 46
days before: $120 admin fee applies. No refunds within 31 days of surgery.

Booking Deposit Refund (less than $400 USD)

No refunds are made with booking deposits of less than $400 USD whatsoever.

All Money Paid to Better Me Bariatrics Policy

No refunds within 31 days of surgery. All payments forfeited. Limited refunds considered
case-by-case, no guarantees. This is due to upfront costs for hotel, transportation, surgery
OR, admin, etc.

Postponed, Rescheduled Policy

No refunds for postponed/rescheduled surgeries. Customers may postpone once with written
notice at least 31 days prior. A second postponement forfeits the initial deposit. Postponing
within 31 days incurs a $500 forfeit and new $500 deposit required.

Doctor Cancellation Policy

If canceled due to not following instructions or false information, all payments are forfeited and
a $1,500 fee applies. If surgery is deemed unsafe intra-operatively, the doctor may cancel. No
refunds in such cases.

Fully-Paid Customers & Remainder Funds Policy

Balance must be paid 7 days prior to surgery. If not paid, surgery is canceled and payments
forfeited. Use wire for last-minute payments. Payment clearance times: Bank/Wire: 3 days,
Zelle: 3 days, Financing: 8 days.

Customer No-Communication Policy

Failure to respond to Better Me Bariatrics may result in canceled surgery and forfeiture of all
funds. Customer still owes any outstanding balances.

Failed Payments, Outstanding Balances Policy

Failed payments incur a $100 fee. Unpaid balances after 30 days are charged 2% monthly.
Collections fees will be added to your balance.
